Summer Camp Parent Agreement 2021

We are so excited to welcome you to Summer Camp 2021! But before jumping in and starting our summer fun we want to communicate our Covid-19 Plan of Operation.  Please see the steps we are taking below. 

Important Note: This plan focuses on reducing exposure and the spread of COVID-19. While this plan is thorough and comprehensive and based on current recommendations, it does not guarantee that a staff member or child will not be exposed to COVID-19 or sickened from it.

Health Screenings:

• Daily temperature checks. We require that you check your campers’ temperature before coming to camp each day. We will also check your camper upon arrival. Any temperature over 100.4 will result in your camper being asked to leave camp. They must be fever free for 24 hours without medication before returning to camp.
• If your camper develops a temperature during camp we will notify you to come pick up your child. You must have a plan in place so your child can be picked up within the hour of being notified.
• Before entering camp your camper must turn in a daily health-screening sheet. They will be provided for you at the sign in table.
• If your child or anyone at home has any of the following symptoms they will need to be cleared by a physician before returning to camp: fever of 100.4or above, new cough, shortness of breath, muscle or body aches, diarrhea/vomiting, new loss of taste or smell.

 Camper Care:

• Campers will enter and exit camp each day through the Kindergarten yard. We recommend parking in the upper lot.
• Campers and counselors are required to wear face coverings while on campus.   
• Campers must follow social distance guidelines and remain 6 feet apart from other campers.
• Staff will provide campers with their own set of supplies for daily activities. Their supplies will be marked and sanitized daily.
• Staff members will routinely disinfect all shared spaces and items (tables, chairs, playground equipment, bathrooms).
• Staff members will plan time for hand washing before and after meals.

Home Support:

• Wash your hands thoroughly with soap and water for at least 20 seconds.
• Avoid touching your eyes, nose, and mouth with unwashed hands.
• Clean and disinfect items that come to and from camp each day.
• Cover cough and sneezes.
• Campers who are sick or have recently had a close contact with a person with Covid-19 are to stay home. 
• Any campers or staff who were in close contact with someone diagnosed with or suspected to have COVID-19 are to home quarantine for 10 days from the date of exposure. 

Returning to School after Presenting with COVID-19 Symptoms:

Any camper or staff member with symptoms consistent with COVID-19 are to stay home and not  go to school or work.

• Individuals are to stay home until at least 10 days have passed after symptoms first appeared and symptoms are improved and no fever for 24 hours.
• “Recovery” means fever is gone for 24 hours without the use of fever-reducing medications and respiratory symptoms (e.g., cough, shortness of breath) have improved.
• Individuals without Covid-19 symptoms, who had a positive Covid-19 lab test, must isolate themselves for 10 days from the date of the positive lab test.  
• A person is considered to be diagnosed with or likely to have Covid-19, if the person has: a) Received a positive lab test for Covid-19, AND/OR b)been informed by a physician that they are likely to have Covid-19, AND/OR c) signs and symptoms that are consistent with Covid-19 (fever, cough, shortness of breath or trouble breathing.)

Those in close contact with student or staff with COVID-19 symptoms:

• Any campers or staff who were in close contact with someone diagnosed with or suspected to have COVID-19 are to home quarantine for 10 days from the date of exposure. 
• “Close contact” is defined as being within 6 feet for more than 15 minutes with someone who has tested positive for COVID-19, or is suspected to have COVID-19.

I hereby assign to Trinity Summer Camp, the right to record my child's voice and likeness for use in media productions. Trinity Summer Camp has absolute right and permission to copyright and/or publish photographic portraits or pictures of my child, in which I may be included in whole or part, or composite or distorted in character form in conjunctions with my own or a fictitious name or reproductions thereof in color or otherwise, made through any media at their disposal for art, advertising, trade or any other lawful purpose whatsoever.

I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used.

I hereby release, discharge, and agree to save Trinity Summer Camp and all affiliates, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said pictures, or in any processing tending towards the completion of the finished product.

I/WE UNDERSTAND that the policy of Trinity Summer Camp is to make no refund whatsoever on registration or tuition fees.  I/WE UNDERSTAND that the registration fee paid for each child is used to offset services rendered in connection with processing my child's application for admission to the camp.

I/WE HAVE READ AND AGREE to pay Trinity Summer Camp all registration, tuition, late fees, and other school related fees for my child(ren) as stated in the Summer Day Camp Information and Registration Packet and in this Agreement.

I/WE UNDERSTAND that all summer camp tuition payments must be made in advance.

I/WE UNDERSTAND that if I have registered my child to attend any day or week of summer camp, I am responsible to pay for that week's full tuition, unless I cancel my child's reservation by the Tuesday prior to the week of attendance. If my child does not attend the summer camp, and I did not cancel the reservation by the Tuesday prior to the week of attendance, I am responsible for paying the full tuition for the day or week which I reserved.  

I/WE UNDERSTAND late fees will be incurred when my child is on school property after camp dismissal at 5:00pm. If a camper is picked up late, I/WE UNDERSTAND that a fee of $20.00 for every 10 minutes or portion thereof will be charged to the family account.  After 3 late pick-ups, a fee of $40.00 will be charged to the family account for every 10 minutes or portion thereof.

I/WE UNDERSTAND my child will not be admitted to the camp with any unpaid late pick-up, tuition or other fees.

IMPORTANT:  Signatures of both parents are required (unless you are a single parent). The parent responsible for the financial obligation to Trinity Summer Camp must have his/her signature on this application.

The philosophy of behavior management and discipline at Trinity is built upon the foundation of the Word of God, which is profitable for training in righteousness (2 Tim 3:16). The goal of this policy is not to set up consequences and punishments for students when they misbehave, but rather to establish positive expectations for student behavior. A second goal of this policy is not only to address the outward behavior of the students, but also to transform their hearts and minds so they understand how to interact with others in a way that pleases and glorifies God. The rules of the camp are reflected in these 3 principles:

1.     Obey God, Parents, and Teachers

2.     Love Your Fellow Students

3.     Respect Property of the Camp and of Others

DISCIPLINARY ACTIONS*

FIRST OFFENSE:   Offense noted and conversation with camp counselor.

SECOND OFFENSE:  Camper will be restricted from some camp activities. A behavior notice will be sent home to the student’s parents to inform them of their child’s behavior at the camp.

THIRD OFFENSE:   Further restrictions imposed which may include exclusion from field trip attendance. A behavior notice will be sent home to the student’s parents to inform them of their child’s behavior at the camp. Conversation with camp director.

FOURTH OFFENSE:  Camper may be removed from camp attendance.

*Any of these steps may be skipped at the discretion of the camp staff based upon the frequency and severity of the camper’s misbehavior.

Release and Waiver of Liability and Indemnity Agreement

In consideration of being permitted to participate in all on-campus and off-campus activities at Trinity Summer Day Camp, the undersigned, for him or herself and any personal representatives, heirs, and next of kin, hereby agrees to the following:   

1.     THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE Trinity

Summer Day Camp/Trinity Classical Academy, its officers, employees, agents, and representatives (hereinafter referred to as "releasees") from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damage and any claims or demands therefore on account of injury or death of the undersigned or property loss or injury, whether caused by the negligence of the releasees or otherwise, while the undersigned is participating in Trinity Summer Day Camp activities, both on and off camp premises and including, but not limited to, field trips to: Color Me Mine, Painting with a Twist, Aquatic Center, and Rockin' Jump. Locations subject to change.

2.     THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage, or cost he or she may incur due to the undersigned's participation in Trinity Summer Day Camp activities whether caused by the negligence of the releasees or otherwise.

3.      THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH, OR PROPERTY DAMAGE due to the negligence of releasee or otherwise while participating in Trinity Summer Day Camp activities.

THE UNDERSIGNED further expressly agrees that the foregoing RELEASE, WAIVER, AND INDEMNITY AGREEMENT is

intended to be as broad and inclusive as is permitted by the law of the State of California and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.
