Trinity Sign-Up Forms

All athletes (elementary through high school) must fill out and turn in a Parent-Student Agreement, a current Physical Examination form as well as a Medical Release form. In addition, all high school athletes must fill out a CIF Code of Ethics form.  All forms are available in the following two links below.
